Consent Form

I give permission for my daughter to participate in the GirlZ to Women™ Retreat sponsored by Zeta Phi Beta Sorority, Inc. Furthermore, I consent to allowing my daughter to participate in discussions on sex education.**

I do hereby grant permission to Zeta Phi Beta Sorority, Inc. to use photos of my child taken at GirlZ To Women™ to be used in marketing materials. I understand (a) the images will be used exclusively to promote the activities of aforementioned organizations, (b) the images will be the sole property of the respective organizations, and (c) there will be no wages or payment of any kind in return for this appearance. Furthermore, I am aware that these photos may be published on the organization’s website; however my child’s personal information (i.e. name, age, etc) will not be made available

**Although a session/discussion is not planned on the topic, if the participants have questions regarding this topic we will defer answers to the trained health care professionals.
